Does Choice Ultrasound bill to or take insurance?
While we don’t bill directly to insurance; we do accept health-savings cards, credit cards and cash payment. We will provide an itemized receipt, complete with CPT and diagnostic codes that most insurance companies recognize for reimbursement which you will need to submit yourself. You can submit a personal claim using our itemized receipt for reimbursement. (Note: We do not guarantee reimbursement by your insurance; so please check with your insurance provider before scheduling your appointment; if reimbursement is important to you.)
Do I need an order form from my doctor to receive a diagnostic ultrasound?
Yes, you can download our order form here. Your healthcare provider will need to fill out the form and fax It to us at (833) 938-4654. We will not be able to render any services without a valid order form completed by your healthcare provider prior to your appointment.
